Your First Visit

Our team loves welcoming new patients to our San Jose dental practice and developing long-term, trusting relationships. We want your first visit with us to be as productive as possible, so here are a few things you need to know before you arrive.

Forms

Life runs on paperwork, and dentistry is no exception. The forms we ask you to fill out give Dr. Pham the critical information he needs to provide you with the safest, most appropriate care.

Before your appointment, please visit our forms page and download and print the new patient packet. This form asks about your medical history, and all medications you are taking that might impact your treatment.

If you can’t complete the forms beforehand, please plan to arrive at our office at least 15 minutes before your appointed time.

Please also remember to bring your government-issued ID and any insurance cards you plan to use.

Meeting Your Team

When you arrive at our dental office, Lydia or Evelyn will greet you and check you in. If you haven’t completed your paperwork, they will get you settled so you can fill out the forms here in our comfortable reception area.

At your appointed time, we will show you back to your treatment room and help you get settled with a blanket or neck pillow. Dr. Pham will then meet with you to discuss what level of care you are interested in and how you would like us to help you. He will perform a thorough exam and dental cleaning and review his findings with you.

Dr. Pham will develop a treatment plan based on your needs and preferences and schedule subsequent dental appointments so you can plan accordingly. At this time, we will also discuss your payment options and how your insurance applies.
